To get reimbursed please fill out the electronic forms below and email them together with your receipts to our Travel and Accounting Coordinator Joyce Sphabmisai (joycesph@ucr.edu). 1) Reimbursement Form.

2) Mileage Report if you drove to/from UCR.

3) Form W-9 or Form W-8. (Use the W-9 if you are a US Citizen or Permanent Resident and use the W-8 otherwise).

4) Attach all itemized receipts.
